Job Description:

Licensed Practical Nurse (LPN) - PRN position in Sandusky, Ohio, offering the opportunity to provide personalized nursing care to older adults.
Deliver direct care by assessing, planning, implementing, and evaluating patient needs based on scientific nursing principles.
Administer medications orally, subcutaneously, or via IV, and document all treatments accurately.
Monitor vital signs, report adverse reactions, and assist with daily activities to promote independence and self-esteem.
Manage common geriatric syndromes across multiple systems and support patients-------- active participation in their healthcare.
Prepare equipment, assist physicians, and involve family members in care when appropriate.
Ensure thorough documentation of all care provided and perform other related duties as assigned.
Qualifications include a high school diploma or GED, graduation from an accredited nursing program, current Ohio LPN license, and BLS certification.
